About this paper:As a Registered Home Therapy Nurse, you will manage patient care in an independent home therapy program by providing education and support to patients and caregivers who have chosen dialysis from the comfort of their own home. This role has a primarily clinical basis. They make an initial visit to patients' homes to assess readiness for home therapy with regular training and follow-up visits to the clinical setting.

MAIN DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policies, procedures, standards of nursing practice, and state and federal regulations.
- As a member of the nephrology healthcare team, you will be involved in decision-making, teaching, leadership, and quality improvement activities that improve patient care outcomes and home therapy program operations.
- Identifies and assesses home therapy program candidates, including assessment of patients' home environment, if necessary.
- Conducts an assessment and identifies barriers in the home environment of the home dialysis candidate and the caregiver's willingness and ability to provide home dialysis treatments.
- Enroll new patients in the Home Therapies program using the appropriate processes.
- Coordinates patient care, including assessment, planning, intervention, and evaluation for an assigned group ofHemodialysispatients
- Evaluates and manages the patient's response to home dialysis training and treatment therapy following prescribed pre-defined protocols.
- Analyze dialysis data for home therapy patients and document and adjust or change treatment plan as needed.
- Provides initial and continuing education for patients and families on home dialysis treatment and the operation and maintenance of all home therapy equipment.
- It guarantees that the patient knows the modalities of transplantation and treatment.
- Administers medication as prescribed or according to approved algorithms and documents appropriate medical justification and efficacy.
- Initiates or supports emergency measures.
- It complements routine patient assessments within the clinical setting.
- Ensures that the correct collection, processing and shipping procedures are carried out in the laboratory.
- Collaborates with the interdisciplinary team on the patient care plan.
- Referrals to social workers and registered dietitians when appropriate.
- Meet CAP requirements to maintain or advance in the program.
EDUCATION:
- Graduated from an accredited nursing school.
- Current relevant state approval.
- Current or successful completion of CPR BLS certification.
- Must meet practice requirements in the state in which he or she is employed.
EXPERIENCE AND SKILLS REQUIRED:
- Qualities and Traits: Compassionate, caring, supportive of colleagues, cooperative, trustworthy.
- Registered nursing experience (preferred but not required).
- Experience in chronic/home hemodialysis (preferred but not required).
- Successfully pass the Ishihara color blindness test.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS:
- The physical demands and characteristics of the work environment described in this document are representative of those encountered by an employee performing the essential functions of that job. - The position provides direct patient care including regular heavy lifting, moving patients and assisting with walking. Equipment handlers and/or staff can provide assistance.
- This position requires frequent and prolonged standing and the employee must be able to bend down.
- Employee may occasionally be asked to help move machinery and equipment up to 200 pounds and may lift chemical and water solutions up to 30 pounds. as tall as 5 feet.
- The work environment is characteristic of a health center with air temperature control and moderate noise levels.
- You may be exposed to infectious and contagious diseases/materials.
- Daily work includes desk work, computer work, interaction with patients, facility/hospital staff, and physicians.
- The position may require travel to home dialysis patients, training centers, or other facilities.
- You may be required to perform the essential functions of this position in other locations with the same physical demands and working conditions described above.
